Your application (and uploaded evidence) needs to demonstrate that you meet the requirement for:
Appropriate training in both implant dentistry and mentoring: your application below should reference the formal courses that you have completed, each of which should be supported by certificates (and where appropriate, transcripts) uploaded on the Evidence Portal.
Appropriate level of clinical experience: your application below should cite manufacturers’ references (which should be uploaded to the Evidence Portal), and you should attach a log book and portfolio below. You may also cite clinical references (again, uploaded to the Evidence Portal).

Remember to complete the separate registration form, which includes payment, and upload evidence to the CGDent online portal, if you have not already done so (you will need to be signed into your CGDent account for both).
